Enrollment Process

If you have recently moved to the Oak Ridge R-VI School District, you can access enrollment and health forms at the bottom of this page. To enroll, you will need to go to the Principal’s office with these following forms:

• Enrollment Forms

• Student Health History Form

• Child’s Official Birth Certificate

• Immunization Record

• Parent/Guardian Driver License

• Proof of Residency
